East One Primary Care Network Health Inequalities Leads, part of the Barking and Dagenham Health Inequalities Programme, have been collaborating with East One PCN care coordinators, Barking and Dagenham Local Authority, East One PCN clinical director, B&D chair for NEL ICB, and the GP Federation to co-design and deliver drop-in clinics at Dagenham Library over a year. The Core20PLUS5 approach to reducing health inequalities and data analytics specific to East One PCN have shaped the following drop-in clinics: hypertension health, women’s health, men’s health, children and young people and asthma health and age 65 plus health.
