WINNER: Boehringer-Ingelheim and Inspira Health, The Care Lab, Widnes PCN, Widnes Highfield Health, and NHS Trusts: A Cardio-Renal-Metabolic Quality Improvement Programme - Delivering Change for Patients with Long-Term Conditions in Widnes
A cardio-renal-metabolic programme in Widnes put the “patient voice” at the heart of service redesign, bringing care closer to home. Addressing the complex relationship between heart failure, type 2 diabetes and chronic kidney disease, the power of real-world data was harnessed to drive improvements in early identification, diagnostics, intervention and service delivery. Medical therapy, glycemic control and cardiorenal protection were also improved. Organisers embedded an MDT approach in a community setting. The programme reduces inefficiencies and improves integration and engagement with diverse stakeholders across the system locking in care closer to home by hardwiring financial flows, general practice and community services in response to patient feedback.
