WINNER: Kumar Medical Centre and Frimley Integrated Care Board: Reducing Health Inequalities in Slough 

Eighty-five per cent of Frimley Health and Care Integrated Care Board’s underserved population lives in Slough, Berkshire. The project focused on the wider determinants of health for a subsection of its population (3,582) living in decile 1-3 with two or three chronic conditions. They were invited to answer the 16-part DiPCare-Q questionnaire to help identify areas of perceived burden such as payment of household bills, lack of food or clothing, mental well being, and digital inaccessibility. Responses and subsequent targeted interventions were all coded digitally to create a powerful dataset that informs the social commissioning offer through richer, deeper insights into patient-reported needs.
