Early Discharge After AMI Pathway
A novel Early Hospital Discharge after Acute Myocardial Infarction pathway was designed and implemented to reduce hospital length of stay, establish optimal virtual clinic follow-up and improve patient experience. Between 2020-2022, 1,000 low-risk patients were discharged on the pathway. The project demonstrated that with an integrated, structured multidisciplinary virtual follow-up schedule it is possible to safely and successfully discharge patients 24 hours after an AMI. The pathway shortened length of stay, decreased the risk of nosocomial infections, optimised resource utilisation and most importantly enhanced the quality of post-discharge care with high levels of patient satisfaction. The judges said this was an impressive and highly innovative entry with clear, clinically led criteria.
