Dynamic Support Database: Clinical Support Tool
Commissioners must keep a database/register of autistic people and people with intellectual disability at risk of inpatient admission (NHS England, 2017). The Dynamic Support Database - Clinical Support Tool is a validated and reliable tool to RAG rate individuals in relation to their risk of admission. This keeps up-to-date information available for the register and ensures inter-rater reliability in relation to the rating. The stratification of need in relation to the highest-tiered provision of service (hospital services) allows services to provide increasing levels of support to reduce the risk of admission. The change approach and the level of spread of this innovation was particularly impressive, as was the approach to collecting impact data. This was an altruistic and compassionate project where the clear aim was to genuinely make a difference.
