The power of collaboration: revolutionising heart failure care through digital innovation and transformed care pathways

Heart failure is increasingly common and incurs a substantial cost in terms of quality and length of life for the patient, as well as fiscally to the NHS. The project worked collaboratively with primary care colleagues to redesign its entire HF pathway to diagnose HF earlier, therefore avoiding admission to secondary care. The combination of prompt detection, early prognostic therapy and remote monitoring via partnership working has reduced costly emergency hospitalisations and significantly improved the health outcomes of patients experiencing HF. This piece of work was a powerful example of collaboration, delivering change for patients.
