NHS Black Country ICB and Walsall Together: The Population Outcomes Framework

ICS/ICP frameworks set out the need for greater collaboration between partners and community to ensure transformation and investment decisions are aligned, effective and deliver improved outcomes.

Black Country Integrated Care Board Academy, Walsall Together and VUIT developed a Population Outcomes Framework and digital tool that would identify and measure evidence-based population outcomes for health and wellbeing, supporting both place and system to measure, monitor and make investment decisions for health inequality transformation programmes at pace.

Walsall’s deployment of the tool has led to partnership initiatives being targeted to population wellbeing outcomes, with progress being measured consistently by all partners to drive transformation.
