Southern Health and Social Care Trust: Acute Care@Home Team - Virtual Monitoring of Care Home Patients
The Acute Care@Home Team has been treating patients with covid throughout the pandemic. It was important that care homes were supported in caring for their covid patients, in the knowledge that patients could deteriorate rapidly in the second week of illness. It was decided that virtual monitoring of these patients was important to identify the deteriorating patient and escalate their care either to a GO or the AC@HT.
