NHS Cheshire and Merseyside: Remote Monitoring Enabled Heart Failure Virtual Ward

NHS Cheshire and Merseyside - and local community trusts - working in partnership with a hospital trust, was the first site in the North West to launch a remote monitoring-enabled heart failure virtual ward, to help reduce hospital length of stay through supported early discharge of appropriate patients and improved admissions avoidance.

Supporting over 300 patients since going live in May 2022, the pathway has demonstrated a 50 per cent reduction in 30-day readmission rates, along with a 50 per cent reduction in 30-day mortality.

This is a truly integrated model using remote monitoring to support heart failure patients on a virtual ward. The judges were impressed with the well-designed intervention, noting its positive impact on resource efficiency and patient experience. They felt the compelling patient testimonial emphasized its value. The presentation showcased the successful implementation of patient-centred care, considering partnerships and achieving all aims with evidence.
