NHS Elect: The Acute Frailty Network - Supporting Older People Living with Frailty to get Home Sooner and Healthier
Older people living with frailty often suffer harm in hospitals, with one study reporting that 76 per cent suffer an adverse event. The Acute Frailty Network (AFN) was created to support people with frailty and urgent care needs to get home sooner and healthier.
AFN networks hospitals across the UK undertake timely delivery of Comprehensive Geriatric Assessment (CGA) in urgent care settings, to reduce reliance on acute hospitals, save lives and reduce functional deterioration. The emphasis is early discharge supported by the wider health and social care system. The primary delivery vehicle is the coupling of evidence-based care with quality improvement.
The judges praised the successful, well-supported programme. The project’s wide dissemination shows positive outcomes, with strong evidence supporting enhanced care for frail elderly individuals.
