Chelsea and Westminster Hospital FT: CCOT Advanced Clinical Practice : Its Impact In Improving The Recognition And Escalation Of Deteriorating Patients In The Adult In-Patient Wards

Sub-optimal care due to failure to recognise and escalate deteriorating patients is a shared global challenge that could result in adverse health outcomes. The Critical Care Outreach Team employed their advanced clinical practice in implementing improvement initiatives resulting in improved monitoring of physiological observations of patients, leading to timely recognition and escalation of deteriorating patients. With the CCOT’s specialised clinical and teaching skills and strong leadership, new concepts in the monitoring and escalating of physiological observations of patients were embedded successfully, contributing to improved patient survival, positive patient experience and other positive health outcomes.
