Emergency departments are the first place someone enters the healthcare system, often in a vulnerable, critical illness state. Capacity issues, relentless high demand on services, increasing ambulance waits and less overall movement through acute hospital wards places additional pressures within the ED to provide care. This is all compounded by competing clinical priorities of those with serious illness.

Development of pressure ulceration, dehydratio, and meeting basic nursing care needs within this patient group awaiting assessment is especially difficult. HOP (hydration, observation, and pressure prevention) was implemented among patients awaiting offloading from ambulances and in the waiting room.
