Conexus Healthcare: GP Care Wakefield

Providing accessible same day access to primary care is one of the keys to controlling accident and emergency attendances. Conexus was commissioned by the clinical commissioning group to work with practices and patients to design a way to offer same day GP and advanced nurse practitioner appointments from two sites.

Patients are triaged before being offered an appointment, if appropriate, and may be given self-care advice or directed to other services. The service runs on weekday evenings and 9am to 3pm at the weekend.

Routine primary care appointments are also available, bookable by patients through their own practices. The service has been highly rated by patients and has been evaluated.

It was developed, designed and implemented over eight months, at a cost of around £215,000: the CCG also supported it through communication and engagement activities. It is a replicable model to the issues of providing access to extended hours primary care and learning will be spread through an online video among other methods.
