Population health management and medicines optimisation are widely acknowledged as central to integrated healthcare, which provides value for money and addresses health inequalities.
But these two concepts are not always considered together. The idea of PHM — using data to drive improvements to the health and wellbeing of a given population, including by tackling inequalities — is often separated from the idea of ensuring that patients are on the best medicines for them.
Could there be value in removing that separation and considering the two concepts together?
In other words, could prescribing and other pharmacy data contribute to the population segmentation and targeted interventions of population health management? On the other side of the coin, could existing population health management projects help bolster medicines optimisation opportunities? Could previously-collected data help identify groups in which polypharmacy might be especially common? Could it assist in identifying patients who have not engaged with services for some time, and who may therefore benefit from a medication review?
This HSJ webinar, run in association with Optum, will explore how these two frequently promoted approaches might complement and nourish one another.

Vicki Kong, Havering clinical lead for population health management and clinical lead for Digital First community pharmacy, North East London
Vicki Kong is an experienced pharmacist, having worked in hospital and in primary care settings for 20 years in North East London. She is currently clinical lead for population health management for Havering, also using her experience to support other boroughs with proactive care. She also provides local clinical leadership for digital in pharmacy for North East London Integrated Care System and recently completed a master’s degree in digital health leadership.
Peter Milmer, clinical product specialist and GP partner, Optum and Honiton Surgery
Peter Milmer is a GP partner in Devon and supports Optum as a clinical product specialist. He has extensive experience in ideas development and deployment, primary care management, and change management.
Minesh Parbat, chief pharmacist, Shropshire, Telford and Wrekin ICB
Minesh Parbat has worked in senior leadership positions across the NHS and has led on medicines optimisation projects both strategically and operationally. He is currently chief pharmacist at Shropshire, Telford and Wrekin Integrated Care Board and has a keen interest in organisational development and collaborative working.
