WINNER: Kaleidoscope Health and Care, Edge Health and Cambridgeshire and Peterborough STP: Establishing Digital Diabetes Care Services in Cambridgeshire and Peterborough

Cambridgeshire and Peterborough STP commissioned Kaleidoscope Health and Care and Edge Health to co-produce and implement a ‘digital first’ type 2 diabetes pathway to support the STP’s ambition for diabetes remission. Kaleidoscope and Edge worked with five primary care networks with the highest levels of deprivation and poorest outcomes. The programme considered ways technology could help address the biggest challenges to improving care and make things more efficient for both patients and staff. These PCNs continue to work together to improve diabetic care and this approach has been rolled out to a further set of PCNs locally.
