The judges said: “This project demonstrated clear clinical benefit and substantial financial benefits. It really shows what can be done in truly integrated care.”

The Lewisham Integrated Medicines Optimisation Service (LIMOS) was created to provide a formal pathway for patients with medicines related problems and those at risk of medicines related readmission.

Delivering patient-centred care is at the heart of this project but through this the team have also been able to make significant cost savings for services in Lewisham. 

LIMOS directly assesses referred patients. The team begins by having a discussion with the patient or carer to gain more insight into the way they use their medicines and also into their mental and physical state. Liaising with the GP, community pharmacist and social service team an integrated pharmaceutical care plan is then agreed. After this, LIMOS provide regular follow ups for one month, following the patient’s review.

In the past 10 months 302 patients have been reviewed by the team. Of those, 111 possible accident and emergency attendances and 19 possible hospital admissions were avoided, and 150 unnecessary medicines stopped.

It is estimated that for every £1 spent on delivering the service £2 is saved across health and social care.
