Warrington Clinical Commissioning Group: Commissioning for Choice at End of Life

The team improved patient care in the community by supporting patients to co-design their end of life care around their needs, wishes and lifestyles.

Eighty two per cent of people were able to die in their preferred place of care.

Judges said that the team demonstrated a clear ability to deliver bespoke compassionate care, despite many challenges along the way.
