Within Lambeth and Southwark, the Patch Children’s Community Nursing service delivers an early intervention nursing service for children and young people with asthma. The service reduces health inequalities in access and outcomes through proactively contacting patients who may have asthma, and offering biopsychosocial treatment to those with uncontrolled symptoms. The service improves quality of care and outcomes, as demonstrated through improvements in asthma symptom control and reduction in health service use. The service was created through the Children and Young People’s Health Partnership collaboratively with families, clinicians, and researchers to enhance care for children.
