WINNER: Sunderland Clinical Commissioning Group – Reaching out to our military veterans and their families, reservists and adult volunteer cadets
Sponsored by
The CCG made identification of veterans and providing the best care to them a priority. Since 2012 it has raised awareness among GP practices, which now all have a named armed forces champion, with same day contact for veterans in need.
Anyone with a military background is now identified within medical records, veterans who are in a crisis when their practice is closed can get access to appointments through an armed forces lead, and a local incentive scheme has been used to increase the number of veterans registered with a GP.
The CCG has worked with Veterans in Crisis to set up a 24/7 space where homeless veterans can access short term beds.
